Instructions for Employers

​

As part of the CSQ application process, you can provide evidence for each employee.​

• Employees may be eligible for CSQ funding if they perform eligible industries and roles.

• You can submit evidence for up to 10 employees at a time.

• If all employees have the same role and work at the same location, you can submit one letter for all of them.

• Download and complete the Employer Letter Word Template before starting.

• Make sure you have all the required evidence documents ready to upload as this form won't save if you  leave it.

• USI Transcript - see instructions on how to download the transcript

• Medicare Card
   

And optionally:

• ​Apprenticeship Training Plan (for apprentices only)

• Visa with conditions (for Visa holders only who have a CSQ Eligible Visa)​
