CSQ Evidence Required - Construction Workers
As part of your application for CSQ Funding you will need to provide evidence to show:
1. You have not done the course before
Provide:

USI Transcript see instructions
2. You are an Australian or New Zealand Citizen or Permanent Resident, or Visa holder with relevant permission to work
Provide one (1):

Green Medicare Card OR Birth Certificate OR Passport (For Australian or NZ Citizen or Permanent Resident)

Employer Letter with Visa # and confirming the right to work in Australia (Visa Holders)

3. You are an eligible Worker in the Building and Construction Industry
Provide one (1) of the following:

Employer Letter

This letter must confirm your employment, role and duties. It must be on letter head and signed by the employer.
Only Queensland licences on this list are approved evidence by CSQ.

QBCC / ESO Licence AND Employer Letter (download template)
CSQ do not accept Interstate Licences as evidence. In that case please provide another form of evidence such as a QLeave Membership Statement or Employer Letter.

QLeave Statement AND Employer Letter (download template)
This must show your name and credits within the last 4 years.
You can get this from QLeave Online Services.

ABN/ACN registration AND Invoice (for self employed)
ABN/ACN registration showing ownership AND a copy of an invoice detailing work undertaken for a client
You can get a copy of your ABN from the Australian Business Register
AND

Statutory Declaration (for unemployed)
Statutory declaration confirming you have been unemployed for less than 4 years (for unemployed construction workers)
This must be signed by a Justice of the Peace.

Once you have your evidence, complete the CSQ Eligibility Form for Construction Workers, Unemployed Construction Workers, or Self Employed.
