EVIDENCE OF IDENTITY

For all High Risk Licence courses - REMEMBER TO BRING your Evidence of Identity documents.
You can bring either:
  • 1 Category A and 2 Category B documents OR
  • 2 Category A and 1 Category B documents
Category A documents
Status
  • Australian Birth Certificate / Bicentennial Birth Certificate (born in 1988) – full (not Extract or Commemorative Certificate)

  • Australian Citizenship Certificate / Naturalisation Certificate

  • Queensland or Federal police officer photo identity card

Current

  • Australian passport

  • Foreign passport

  • Australian photo driver licence*

  • Queensland Government issued photographic high risk work licence

  • Australian Certificate of Identity, Document of Identity, Titre de Voyage, Document de Voyage

  • Queensland Accreditation / Authorisation: driver/rider trainer; pilot/escort vehicle driver; dangerous goods driver; tow truck driver/assistant; traffic controller; passenger transport driver (bus, taxi, limousine)

Current or expired less than two years

  • DIBP – ImmiCard. Note: Electronic Travel Authority (ETA) is not accepted*

Valid up to five years after issue

  • Queensland Card 18+ (laminated)

Issue after 01/01/1992

Category B documents – must be Australian issued documents
Status
  • Financial institution debit/credit card (must include signature and embossed/printed name)*

  • Medicare card; Department of Human Services BasicsCard*

  • Australian Defence Force photo identity card (excluding civilians)

  • Australian Firearm Licence (with photo)

  • Australian Security Guard/Crowd Controller Licence (with photo)

  • Department of Veterans’ Affairs/Centrelink Pensioner Concession Card (including Healthcare card)*

  • Education institution student identity document (must include photo and/or signature)

  • Interstate government-issued or government-approved photo identity card

* Please Note:
  • Digital EOI
    The only form of digital EOI that is accepted at this point in time are:
    • Qld Digital Licence when it is displayed and verified in the digital licence app (Cat A).
    • Digital Medicare card and Department of Veterans’ Affairs or Centrelink Pensioner Concession card (including Health Care cards) are acceptable when it is displayed in the Medicare Express Plus mobile app, myGov website or myGov app (CAT B).
Screenshots of these digital documents are not acceptable.
  • Interstate photo driver licence
    This must be in the physical form to be acceptable.
  • Department of Immigration and Border Protection (DIBP) – ImmiCard: Temporary or Resident Visa
    Only the physical Immicard not expired more than 5 years is acceptable.
    Electronic temporary authority (ETA) and other temporary travel documents such as temporary visas are not accepted.
  • Financial institution cards 
    Where individuals are unable to provide sufficient CAT B documentation, multiple bank cards may be used including more than one from the same institution.
    The bank card may also be from an overseas bank as long as the name is imposed on the card and the institutions name is on the card.
  • Change of Name
    If an individual has changed their name, and the name on the Evidence of Identity document is different to the name they want the High Risk Work Licence to appear in, the assessor will need to sight and retain a copy of a change of name document.
